Stakeholder Feedback Survey
Anonymous survey for stakeholders to feed back on a project, team, or change — short, specific, useful.
When to use this
Five questions, deliberately. Long surveys get low response and lower-quality answers. Each question is action-oriented: 'what should change' beats 'how do you feel'.
